I am designing a Near Death Experience poll for PHPBB. So far, I plan to have the following questions in a “select all that apply” format. Any thoughts, ideas, or questions?

I am taking this poll.

I took this poll elsewhere. I am taking it again.

I have personal recollection of leaving this physical body.

I have personal recollection of finding myself outside this body and in another world.

Trauma has caused me to spontaneously find myself outside this physical body.

While outside this physical body, I have noticed others living in this physical world.

While outside this physical body, I have noticed others not living in the physical world.

While outside this physical body, I have communed with others.

While outside this physical body, I have experienced a realm of darkness. (Hell, Void, or Tunnel)

While outside this physical body, I have gone toward a source of light.

While outside this physical body, I have entered a source of light. (Oneness)

While outside this physical body, I have seen, recalled, or experienced my physical life and/or relationships. (Life review)

While outside this physical body, I debated the idea of returning.
